Cristi Leeberg is a dedicated nurse practitioner serving Brevard County, providing compassionate primary care since 2011. She is the founder of Practitioners in Motion, PLLC (PIM), established in 2015, a practice focused on acute and chronic care management for patients in assisted and senior living communities, with a special emphasis on geriatric care.
With over 25 years of experience in nursing, Cristi brings a wealth of knowledge from diverse areas, including acute care rehabilitation, post-operative surgical and trauma care, sub-acute neuro care, home health program direction, and primary care internal medicine. She earned her postgraduate nurse practitioner degree from Ball State University in Indiana and is board certified in adult primary care.
In addition to her clinical work, Cristi has contributed over 15 years to the Brevard County Examinee Committee for the 18th Judicial System, providing professional evaluations for capacity, and continues to serve as an expert witness when called upon. She remains an active member of the Space Coast Guardianship Association.
Outside of her professional life, Cristi enjoys the company of her two adopted rescue dogs, relaxing to the sound of ocean waves, and spending quality time with friends and family. Her passion for care extends beyond her patients—she truly believes in nurturing well-being in every aspect of life.

Robert Johnson is a nurse practitioner with over 17 years of healthcare experience, specializing in Adult and Gerontology Primary Care. He is board certified by the American Academy of Nurse Practitioners and earned his degree from the University of Cincinnati.
A proud Army Veteran, Robert served honorably as a United States Army Medic for 10 years, gaining experience both stateside and abroad. His healthcare journey also includes roles as an EMT, LPN, and RN, fueling his passion for providing compassionate, high-quality care as a nurse practitioner. His diverse clinical experience spans Emergency Medicine, Intensive Care, Post-Surgical Care, Neurological Care, Cardiac Care, and Medical/Surgical Care.
A lifelong advocate for continued education, growth, and wellness, Robert is committed to delivering the best care possible to his patients. As a Brevard County resident for over 28 years, he is dedicated to providing affordable, individualized healthcare to the community he calls home. Michele is an Autonomous Adult Geriatric Nurse Practitioner, board certified by the American Academy of Nurse Practitioners. She embarked on her nursing journey as a second career after her youngest left home—and she’s proven that it’s never too late to follow your passion. Michele has built a rich and diverse career, using her wide-ranging experience to make a meaningful difference in the lives of her patients.
Originally from the suburbs of Philadelphia, Michele earned her Bachelor of Science in Nursing from West Chester University. She’s cared for patients across Med-Surg, ICU/COVID, Critical Care, and Cardio-Vascular PCU units—but her heart truly lies in Hospice and Palliative Care. Michele brings compassion, expertise, and clarity to patients and families navigating complex health journeys, whether it’s managing a new diagnosis, recovering from illness, living with chronic conditions, or preparing for end-of-life care.
When she’s not caring for patients, Michele is cheering on her beloved Philadelphia Eagles—GO BIRDS!—or enjoying her downtime reading, relaxing, and soaking up the sun by the beach.

Rita Ostendorp is a board-certified Family Nurse Practitioner through the American Academy of Nurse Practitioners. A proud Florida native, she was born and raised in North Titusville, where she continues to call home.
Rita brings over 13 years of healthcare experience to Practitioners in Motion, with a special passion for hospice and palliative care. She believes that quality of life matters above all and is dedicated to ensuring every patient receives compassionate, personalized care. Rita sees it as a privilege to make a meaningful difference in the lives of her patients, knowing that even a kind word or a smile can go a long way.
Outside of her clinical work, Rita is an avid reader, exploring everything from science fiction to the latest evidence-based healthcare research. She enjoys spending time with her husband, children, and animals, staying active through workouts, listening to music, and taking her Kawasaki Ninja out for a ride when the weather permits.

Melissa Simmons is a board-certified Adult-Gerontology Nurse Practitioner (AGNP-C) through the American Academy of Nurse Practitioners, bringing nearly 20 years of nursing experience to her patients. Her career has spanned skilled nursing, telehealth, progressive and intensive care, and now primary care. With a strong foundation as a registered nurse and a genuine passion for preventive care, Melissa is dedicated to helping adults and older adults live healthier, fuller lives.
Melissa began her nursing journey in 2006 as an LPN and advanced through Seminole State College’s LPN-to-RN bridge program in 2014. She earned her BSN from Western Governors University and her Master of Science in Nursing from Florida A&M University, where she completed the Adult-Gerontology Primary Care Nurse Practitioner program.
Originally from New York City and raised in the suburbs of Buffalo, Melissa now calls Brevard County, Florida, home, where she lives with her husband and two nearly grown children. A dog lover at heart, she enjoys cooking, soaking up the sun, and staying active in her free time. Melissa brings warmth, expertise, and compassion to every patient she sees, treating each individual with care and respect.

Matthew Wilson is a Licensed Mental Health Counselor with over 13 years of experience serving the Brevard County community. A proud resident of Brevard for more than 20 years, Matthew is originally from Baltimore, Maryland—and he’s still cheering on the Ravens from afar! A third-generation counselor, he holds a Master’s Degree in Mental Health Counseling from Webster University and a Bachelor’s Degree from Florida Atlantic University.
Matthew works with individuals across the lifespan, creating a safe, supportive space that embraces diversity and encourages authentic self-expression. Known for his genuine and kind approach, he helps clients build resilience, navigate challenges, and achieve personal growth. Grounded in evidence-based approaches such as Adlerian Therapy, Cognitive Behavioral Therapy, Mindfulness, and Interpersonal Therapy, Matthew is committed to providing compassionate care tailored to each client’s unique needs.
Outside of counseling, Matthew enjoys spending quality time with his wife and two sons, exploring new adventures, and embracing just about anything that sounds fun.

She earned her LPN certification from Brevard Community College in 1998 and joined the Practitioners in Motion team in 2023.
